  First line chemotherapy in metastatic colorectal cancers: A review by Alfredo Colombo in Journal of Clinical Case Reports Medical Images and Health Sciences Abstract   A common malignancy that is detected is colorectal cancer (CRC). Unresectable, metastatic colorectal cancer (mCRC) patients have a poor prognosis and typically receive palliative care. Chemotherapy is the mainstay of therapy, but as molecular biology and its heterogeneity are better understood, the field is evolving. Depending on the clinical patient characteristics and tumor molecular profile, first-line therapy uses a combination of chemotherapy and targeted therapies.   Recanalized Umbilical Vein in a Patient with Cirrhosis by Dr. Sreeram Parupudi in Journal of Clinical Case Reports Medical Images and Health Sciences Clinical Image   41-year-old man with history of alcohol associated cirrhosis complicated by ascites presented with hematemesis. On admission he underwent a CT angiography which revealed portal hypertension manifested by a recanalized paraumbilical vein and portosystemic collaterals. EGD revealed Grade II esophageal varices and portal hypertensive gastropathy. A recanalized umbilical vein is a commonly reported finding on imaging in patients with portal hypertension and can be seen in up to 30% of patients with cirrhosis [1]. In adults without portal hypertension the umbilical vein is not visible and forms the round ligament [2].  Serendipitous neurotransmitter findings in a frontline defender participating in a magnetic resonance spectroscopy study of first responders and military personnel with PTSD by David Crompton OAM in Journal of Clinical Case Reports Medical Images and Health Sciences Abstract We report a case study of a patient with a clinical diagnosis of Post-traumatic Stress Disorder (PTSD) referred to a neurochemistry Magnetic Resonance Spectroscopy (MRS) study. The clinical evaluation prior to scanning suggested the presence of PTSD, Major Depressive Disorder (MDD) accompanied by suicidal ideation. MRS prior to and at the completion treatment and 12-weeks post treatment identified neurotransmitters consistent with a diagnosis of PTSD and MDD. The neurochemistry evaluation immediately post-treatment identified levels that compared to healthy subjects while at 12-weeks posttreatment the neurochemistry reflected a recurrence of depressive symptoms that were described by the patient. This has not yet been sufficiently taken into account in diagnostic and therapeutic processes. The aim of this overview is to show, on the basis of the rich tradition of the Christian Orthodox faith, not only that there is a history of Far Eastern provenance with regard to relaxation techniques and mental strategies, but also that the inclusion of spiritual and pastoral elements can help the individual patient to reach “peace of mind” (hesychia).
